Consent to the processing of personal data

The texts of the consents a user gives by ticking the relevant box in the website forms.

1. General terms of consent to the processing of personal data

By ticking the consent box and submitting a form on the website drkamilgadzhiev.ru, the user, acting freely, of their own will and in their own interest, gives consent to the operator — Kamil Farukhovich Gadzhiev (address for written requests: 56 Michurinsky Prospekt, bldg. 1, Moscow 119192; email: zero0564@mail.ru) — to process their personal data under the terms of the relevant section of this page. The processing procedure is described in the Personal Data Processing Policy; acknowledging it is not part of the consent.

Actions with personal data: collection, recording, systematisation, accumulation, storage, clarification (updating, changing), retrieval, use, provision (access) to persons authorised by the operator, blocking, deletion, destruction. Method of processing — automated processing.

Personal data is processed on the operator's instructions (Article 6(3) of Federal Law No. 152-FZ of 27.07.2006 “On Personal Data”) by persons authorised by the operator — the consultation coordinator and the managers who handle requests — under instruction agreements and subject to the duty of confidentiality. Personal data is not provided to any other persons, except in the cases provided for by Russian law.

The website forms do not ask for data concerning health. If a user provides such data on their own initiative, it is used only to consider their request and is not published without their separate written consent.

The website, its database and file storage are hosted on computing resources provided to the operator by OOO "Beget" (TIN 7801451618; office 726A, 8B Karla Faberzhe Square, St Petersburg, 195112, Russia) under a services contract; the servers and databases are located in the Russian Federation; website data backups are stored in encrypted form in private object storage provided by OOO "Beget" in the Russian Federation for no longer than 30 days (Sections 9 and 12 of the Policy).

The consent is valid until the purposes of processing are achieved or until it is withdrawn; storage periods for each type of data are set out in section 9 of the Policy.

The consent may be withdrawn at any time by a statement sent to zero0564@mail.ru or to the address for written requests. After withdrawal, processing stops and the data is destroyed within 30 days of receipt of the withdrawal, unless its retention is required by Russian law.

Consent to the processing of personal data and consent to the dissemination of personal data are given separately from each other.

2. Consultation request

Personal data: name; phone number; the interface language and the address of the page from which the request was sent.

Purposes of processing: to contact the user by phone regarding their request — to clarify the request, agree on a time, confirm the consultation and settle matters directly related to the request. This consent is not consent to receive advertising.

The request form is not intended for diagnoses, information on illnesses or medical documents.

3. Review

Personal data: name; rating; review text; optionally — the procedure, a photograph and a note that the author would like the review published.

Purposes of processing: to consider the review and pass it on to the doctor; if publication of the review is permitted in a written document signed by the patient — to check that this document exists and prepare the review for publication within the scope of the permission.

This consent is not consent to dissemination: a review is published only on the basis of a document signed by the patient (section 8 of the Policy).

4. Question to the surgeon

Personal data: name; procedure; question text.

Purposes of processing: to consider the question, prepare the doctor's answer and publish the question with the answer on the website without the author's name — on the basis of a separate consent to dissemination (section 7).

5. Application to the Case of the Month project

Personal data: name; phone number; one or several categories (face, breast, body); a short description needed to get in touch; confirmation that the applicant is 18 or older; the application number and status.

Purposes of processing: to consider the application to the Case of the Month project, contact the applicant and report the application status; if the applicant is selected — to execute a separate written consent with them for the publication of materials.

The form is not intended for photographs or detailed medical information. The application is not published. Declining to take part in the project does not affect the ability to book a consultation and receive medical care.

6. Conditions of dissemination of personal data

Consent to the dissemination of personal data is given separately from consent to processing (Article 10.1(1) of Federal Law No. 152-FZ). In the website forms such consent can be given only when submitting a question to the surgeon — by ticking a separate box (without it the question is not sent, as the doctor's answer is given by publication on the website). By ticking it, the user gives consent to the operator — Kamil Farukhovich Gadzhiev (address for written requests: 56 Michurinsky Prospekt, bldg. 1, Moscow 119192; email: zero0564@mail.ru) — to disseminate the personal data listed in section 7 by publishing it on the website drkamilgadzhiev.ru. Other platforms (social media, advertising materials) are not covered by this consent.

Reviews, patient materials and Case of the Month materials are published not on the basis of boxes ticked in the website forms but on the basis of a written document signed by the patient, within the categories and platforms permitted by them.

The user sets no prohibitions on the transfer of personal data to an indefinite range of persons or prohibitions and conditions on its processing by an indefinite range of persons unless they have sent them to zero0564@mail.ru. The user may set such prohibitions and conditions; refusal to accept them is not permitted (Article 10.1(9) of Federal Law No. 152-FZ). Information on prohibitions and conditions set is posted in this section no later than three working days after it is received (Article 10.1(10) of Federal Law No. 152-FZ).

The consent is valid until withdrawn. Dissemination stops at any time at the user's request sent to zero0564@mail.ru or to the address for written requests. The request must contain the surname, first name, patronymic (if any), contact details (phone number, email address or postal address) and the list of personal data whose processing is to be stopped. The consent ceases to have effect from the moment the request is received; the material is hidden from the website without undue delay.

7. Question to the surgeon — consent to dissemination

Personal data permitted for dissemination: procedure; question text — together with the doctor's answer. The author's name is not published.

Place of dissemination: the website drkamilgadzhiev.ru, questions to the surgeon section. Publication — after review and the doctor's answer.

Without this consent the question is not sent: the form does not ask for contact details, and the author sees the doctor's answer only on the website. A personal question can be asked through the consultation request form.
